Added pull request template.

Making following assumptions:
* Pull requests do not need to originate from Issues discussion.
	* But encouraged
* No current CONTRIBUTING.md file
	* honor code that people follow current coding conventions
* Tests run and passed
	* New features require additonal tests for confirmation
