9bcca1a7b0 Dropped customized doc template 
It was meant to be used only by the website, not for the doc shipped
with the package.

e5e8cec350 Added table of sqlstate exceptions in the docs 
Note that the column-spanning cells break text docs. I don't think
anybody cares about them, so going to drop them.
